Whip egg whites with powdered sugar, grated lemon zest (if desired) and a pinch of salt until stiff. Bring the milk to the boil, skim off the foam, add a little salt and sugar. Form dumplings from the mixture with a spoon, put them into the milk and let them steep without a lid, but DO NOT boil them up! Lift out the dumplings, arrange on a bed of elderberries and pour over the liquid caramel.
